This class will take the participant through the progression of an incident and how data is collected, evaluated and re-constructed in order to create the sequence of events that led up to the occurrence of the incident.
COURSE OBJECTIVES
? To introduce participants to the needed fundamentals of conducting investigations post incident, and provide an understanding to the common accident investigation methods used.
? Provide Basic Incident Investigation Skills - Steps in Investigating Accidents, Interview(s) processes, documenting aspects and recordkeeping.
? Gain knowledge on how and when to report/record incidents - Preparing reports that withstand legal scrutiny
? Understand the importance of “Root Cause Analysis” and how it is done across the board with any branded program.

1. Terminology and Incident levels (organization specific)
2. Breakdown of steps to an investigation
3. Roles and responsibilities
4. Investigation process – (data collection, statements, interviews, photos and additional evidence)
5. Interviewing techniques
6. Data and evidence analysis and re-construction
7. Report writing and recording incidents
