Care Management Certificate Program

Build the skills to deliver high-quality, person-centered care for hospice and high-needs populations. This 11 CE credit, 2-day, live virtual certificate program prepares care managers to assess, plan, and coordinate care that improves outcomes, enhances patient and family experience, and supports value-based care.

Participants will gain practical tools in comprehensive assessment, care planning, interdisciplinary collaboration, and patient engagement, aligned with national case management standards and real-world practice.

The National Partnership for Healthcare and Hospice Innovation (NPHI) is a member-driven organization dedicated to advancing high-quality, interdisciplinary care for individuals with serious and advanced illness through whole-person, patient-centered approaches that improve outcomes, enhance patient and family experience, and reduce unnecessary utilization across the healthcare continuum. The Care Management Certificate Program builds on this mission by equipping hospice and palliative care professionals with structured, evidence-based competencies that strengthen care coordination, improve clinical and fiscal outcomes, and support success in value-based care environments. The program is being launched to address an opportunity to enhance formal care management training within hospice. Designed in collaboration with nationally recognized experts in care management, the curriculum is grounded in established educational frameworks, aligned with national certification standards, and informed by recent evidence. It is intended not as a critique of existing practice, but as an inclusive effort to support the workforce, build shared competencies, and advance more consistent, high-quality care across hospice and palliative care settings.
