This registration is only intended for licensed professionals. If you are not licensed, please complete a Red volunteer registration. You will be asked to re-register if you complete the incorrect form.

If you are a licensed professional and agree with the terms below, please proceed to register. 

Note: registrations are non-refundable. 

RELEASE & INDEMNITY

WHEREAS the organizer, GYC Canada, is a not-for-profit organization that provides opportunities for individuals to participate in doing evangelistic and humanitarian work;

AND WHEREAS all opportunities for participation are provided on a cost-basis, and all participants join on a voluntary basis;

AND WHEREAS the undersigned has requested that the organizer (GYC Canada) and the partners (Ontario Conference of Seventh-day Adventists, iHeal, and It Is Written Canada) facilitate and permit the undersigned to perform evangelistic work;

AND WHEREAS the mission project is provided under the auspices of GYC Canada; And WHEREAS GYC Canada, the Ontario Conference of Seventh-day Adventists, iHeal, and It Is Written Canada, wish to expressly disclaim any liability for any and all damages with respect to any participant and volunteer up to and including personal injury or death;

NOW THEREFORE in consideration of the agreements herein contained and for the purpose of inducing GYC Canada, the Ontario Conference of Seventh-day Adventists, iHeal, and It Is Written Canada to arrange and facilitate and permit such travel and humanitarian work, THE UNDERSIGNED HEREBY AGREES:

A. to release, hold harmless and indemnify GYC Canada and its respective heirs, successors, assigns, officers andemployees, and the Ontario Conference of Seventh-day Adventists, and their respective heirs, successors, assigns, officersand employees, and iHeal, and their respective heirs, successors, assigns, officers and employees and It Is Written Canada,and their respective heirs, successors, assigns, officers and employees against any liability arising in any mannerwhatsoever up to and including personal injury and death and incurred by any of the undersigned by reason of theundersigned having undertaken such travel and/or work;

B. to release, hold harmless and indemnify GYC Canada and its respective heirs, successors, assigns, officers and employees, and the Ontario Conference of Seventh-day Adventists, and their respective heirs, successors, assigns, officers and employees, and iHeal, and their respective heirs, successors, assigns, officers and employees, and It Is Written Canada, and their respective heirs, successors, assigns, officers and employees for any personal injury or death to any minor person permitted to volunteer by the undersigned;

C. that GYC Canada and its respective heirs, successors, assigns, officers and employees, and the Ontario Conference of Seventh-day Adventists, and iHeal, and their respective heirs, successors, assigns, officers and employees, and It Is Written Canada, and their respective heirs, successors, assigns, officers and employees shall not be liable for and is expressly released from liability or damage caused to the personal property of the undersigned including personal injury or death of the undersigned whether caused by the gross negligence of GYC Canada or otherwise;

D. and to undertake any risks associated with participation in GYC Canada missions under the auspices of GYC Canada and waives any remedy or entitlement on account of the same against GYC Canada, the Ontario Conference of Seventh-day Adventists,iHeal, and It Is Written Canada, and their respective heirs, successors, assigns, officers and employees.

Agreement on Roles for Health Professionals & Service Volunteers

1. Health professionals and service volunteers will be responsible for bringing their own tools/instruments/equipment as requested by their respective coordinators;

2. Will serve only guests that have signed the waiver upon registration (must be wearing the designated wrist band);

3. Are expected to share their knowledge about living a healthy lifestyle whenever applicable, as well as provide with relevant & reliable resources whenever applicable;

4. Are expected to perform (if certified) or notify his/her respective coordinator for first aid if needed;

5. And will be allowed to promote their practice/business, but neither charge for any services they provide during the event, nor regard the event guests as their first-time patients/clients.

Agreement on Practices for Dentists/Hygienists

1. Dentist/hygienist volunteers will keep on file all intake forms of individuals who have received dental services during theCompassion Health 2018 for 10 years;

2. Will serve only those that have been screened with government-issued ID’s;

3. Will be responsible for bringing their own equipment (unless discussed with the organizers of the event);

4. And will be responsible for any injury that may arise from the dental procedures.
