Give yourself and your family peace of mind.
None of us like to think about serious illness or the end of life, but planning ahead can bring comfort and clarity when it’s needed most. This free workshop will guide you through the steps of advance care planning so your loved ones will know how to honor your wishes when it matters.

We’ll help you:
• Reflect on what’s most important to you in times of serious illness.
• Identify who you trust to make medical decisions on your behalf.
• Start meaningful conversations with your loved ones about your choices.
• Complete an advance directive (if you feel ready) and have it witnessed or notarized onsite.

You don’t need to have all the answers before you come—just the courage to take the first step.

Before you attend: Think about who you would trust to speak for you if you couldn’t. It’s important to choose someone who can honor your wishes, even if it’s difficult. Talk with that person about your values and preferences.
